Please fill out the below forms to redeem your Active & Creative Kids vouchers.

• No vouchers will be accepted for redemption without completing the below form.
   

• Please ensure that you have checked the name, date of birth and voucher numbers are exactly how it appears on your voucher before submitting.
   

• Your Active or Creative Kids voucher will be applied to your account upon the funds being received from Service New South Wales. Once the payment has been received/cleared it will reflect upon your Term Account. Please allow up to 2-3 weeks for payment to appear on your Term Account.
   

• No refunds are to be given for vouchers for change of mind and are to be used toward tuition only, no apparel or uniform requirements.
